All clinical laboratory specimens (i.e. human specimens to diagnose a disease or condition) must be ordered by a physician licensed to practice in the State of California. Specimens must be accompanied by a requisition with the ordering physician’s name and the test(s) ordered. Patient identification and billing information must also be provided on the requisition. Sample collection kits are available from the laboratory with collection materials and instructions on how to properly collect and preserve specimens.
